The exam

Built for the OTR exam as it has run since January 2024

Since 2 January 2024 the NBCOT OTR exam has 180 items in 4 hours. The clinical simulation test (CST) items were retired and replaced with scenario sets. Scores are reported on a 300-600 scale, and 450 is the passing standard.

Scenario sets

An opening scene describes a client and setting, followed by four related multi-select items with six options each. You carry the case in your head across all four.

How we practise them

Our OTR scenario sets follow that shape (select 3 of 6) and award partial credit, so you can see which part of your reasoning held up. COTA candidates get six-option multi-select practice too.

How does the free trial work?

Your trial starts when you create your account and lasts 72 hours. Try the full practice bank, simulations, study plan, analytics and flashcards, with up to 8 AI tutor requests per day subject to shared service limits. No card is required and you will not be charged automatically. Choose a one-time pass when the trial ends to keep studying. Your history and account remain available.

Is OT Prep Lab affiliated with NBCOT?

No. OT Prep Lab is an independent study tool. It is not affiliated with, endorsed by or sponsored by NBCOT®, and we have no access to live exam items.

Are these real exam questions?

No. Every item is original, written to the NBCOT OTR and COTA content outlines and grounded in OT textbooks. We do not use recalled or copied exam content.

How many questions are there?

Hundreds of original items today. New items become available after quality review. We put our effort into explaining every option and matching the current exam format.

How accurate is the readiness estimate?

It is an estimate, not a prediction. It places you on the 300-600 scale with an uncertainty band that is wide when you have answered few questions and narrows as you do more. Treat it as one signal among several.

Can I trust the AI tutor?

The tutor answers from textbook passages and shows the PDF page it used, so you can check it. Like any AI it can be wrong. If something looks off, check the cited page and report it. Daily message and shared service limits apply.

Does it cover both OTR and COTA?

Yes. Choose your exam when you set up your study plan. OTR candidates get scenario sets; COTA candidates get six-option multi-select items and the 190-item simulation.

Is this a subscription?

No. Every pass is a one-time payment for a fixed number of days. Nothing renews automatically.

What is your refund policy?

If you ask within 7 days of buying and have answered fewer than 50 questions on the pass, we refund you in full.

How does the pass promise work?

Complete at least two full-length simulations during your pass. If you then sit the exam and do not pass, email support and we will extend your access for 6 months at no charge. It comes with the Until you pass plan.
